Nominee forms are now available to 22nd Annual Employee of the Year Luncheon

Nominee forms are now available to the DFW Hospital Council Foundation’s 22nd Annual Employee of the Year Luncheon. The event is scheduled for Wednesday, April 25 at the Hurst Conference Center in Hurst, Texas.

Last year, 15 recipients were honored from a pool of more than 130 nominees. This year’s theme is “Caregivers Matter: A Survivor’s Story,” with Rebekah Gregory, an inspirational survivor of the Boston Marathon bombings, serving as keynote speaker.

One nominee is allowed in each category per hospital. Deadline to submit nominees is Wednesday, March 14, 2018. Please click the links below to download the nominee forms:
