As part of an effort to reduce readmissions, the Dallas-Fort Worth Hospital Council Foundation coordinated a series of meetings February 27-28 at area hospitals with Pat Teske, a national expert from Cynosure Health Solutions, providing the education.

Teske is the improvement advisor and implementation officer at Cynosure, and with the DFWHC Foundation in tow, did site visits and classes at Baylor University Medical Center, Methodist Dallas Medical Center, Baylor Scott & White Fort Worth, Baylor Scott & White Temple and JPS Health Network.

During the visits, Teske worked with the hospitals one-on-one examining their data and strategies to find ways to reduce 30-day unplanned readmissions. She shared best practices and Cynosure strategies used around the U.S. to reduce readmissions in large teaching facilities.
